Before the surgery, the areas for suction are marked carefully in an erect position. The most commonly used method is the “Tumescent” technique, but sometimes we can use the “super-wet” approach.
A solution of local anesthetic, some drugs, and saline is injected into the marked areas. The surgeons make a small cut of 0.5 – 1 cm in well-concealed areas, usually within skin creases, so they are not noticeable.
The plastic surgeons insert suction cannulas of different sizes depending on the area to be contoured and suck the fat out. The suction is done in a manner that wrinkles and grooves are not seen, and it doesn’t produce any areas of depression. The edges are smoothened out using a smaller cannula. Though it is a blind procedure, the surgeon’s hands guide the cannula to regions that need suction. Here the surgeon’s expertise comes into play.
The removed material usually consists of 80-85% fat, the remainder being blood, plasma, and injected fluid. The same procedure is repeated in all the areas to be sucked to give a contoured finish.
Mega Liposuction
Mega Liposuction is a procedure where the aspirate is more than 5 liters at a time and is becoming more relevant in today’s context.
This procedure is better suited for overweight people who are well motivated, maintain a steady weight pattern, have a proper exercise and diet schedule, and are not in the morbidly obese category.
Sometimes multiple sittings may be required if the patient is obese. Mega liposuctions are not for everyone, and not all surgeons perform them. It requires a team approach to help reduce the duration of surgery. Also, the anesthetists must be trained to monitor patients intraoperatively and postoperatively. This procedure usually requires hospitalization.
This procedure is coming into vogue because it helps the patients kick-start their weight loss journey by taking care of the initial reduction of specific areas, which allows them to follow their exercise schedules more regularly.

Recovery from Liposuction
Pain is usually present for a day or two after surgery. Snug or tight compressive dressings are applied for 6 to 8 days to help in shrinkage of the skin, to decrease swelling, and prevent any blood or serum collections.
In most instances, the patient must wear a pressure garment or girdle for several weeks to months during the healing period. This helps reduce the skin’s swelling and shrinkage to its new position. One might feel localized areas of hardness or firmness in the suctioned areas for 2-4 months following surgery.
During the healing period, numbness or discomfort may be present for variable periods. Sometimes bluish-black discoloration is found, which may last up to two to three weeks following surgery. Swelling takes two to three weeks to come down, and it takes two to four months for the swelling to disappear completely and the final results to be seen.
